What To Bring
A picture ID Your insurance card Your referral or script if applicable Any lab test, medical records, X-rays, or the like that is pertinent to your current medical care A check, cash, or credit card for co-payments, deductible, and co-insurance amounts due before your visit A list of all the medications you are taking
When To Arrive
Please arrive in our office 15-20 minutes early, as we will need you to verify your address and insurance information. We will also need to make copies of your insurance card and identification. This will allow our staff to complete the preparation of your medical forms and records as necessary.
